Purple Poppy Walk for Animals Registration

The Purple Poppy Walk For Animals, is a chance for you to get pledges/donations by walking with your own pets, service animals, or a picture of a beloved pet or honor a pet that touched your heart in some way. Money raised will go directly to support the National Service Animals Monument (NSAM), authorized by Congress to be built in our nation’s capital. Please check yes to the following to indicate agreement. A yes answer is required to participate for statements regarding safety and liability.

I agree to keep my animal under my control at all times. Dogs must be friendly and on a 6ft NONRETRACTABLE leash and mini horse service animals on short reins. All other animals must be in a carrier or stroller. Please make sure your animal is safely secured.*

I agree to care for my own animals needs, up to date vaccinations for your area, water, food, waste bags, hot weather gear, cold weather gear or any other needs your pet may have.*

I agree to not share any personal information regarding pledges I may collect on behalf of the National Service Animals Monument. Walkers may write checks or go directly online to make payments after the event.*

I agree to share any pictures I take and upload to the NSAM website or share with Event Host. NSAM has permission to use my pictures of me and my animals on their website and any social medial for NSAM promotions. I will give NSAM the photographer’s name and location, animal(s) name(s). The photographer holds all copyrights.*

I would like future updates from the National Service Animals Monument to keep me posted on their progress.*

I agree that I will submit a minimum of $15.00 to participate at the end of the WALK along with any pledges or donations I may get on behalf of NSAM. I have 10 days after the WALK to submit any and all funds collected, or encourage people to make donations directly through NSAM website.*

Release of all Liability - PARTICIPANT(S) AGREE TO FULLY RELEASE AND DISCHARGE NSAM FROM ANY LIABILITY for any damages, injuries or claims, including any injuries or damages to dog and other animal or pet walkers or other person or property, as a result of Participant’s involvement in the Walk. Any claims made against NSAM must be brought in California.*
